Health Coach Contract

An agreement from Northwind Studio covering the health coaching program, sessions, and payment terms.

Parties and Overview

This agreement is entered into by Northwind Studio (the "Coach") and Acme Co (the "Client") for the coaching program described below. The Coach is not a licensed medical professional and does not diagnose, treat, or prescribe.

Objectives

• Set baseline health goals during intake.
• Build lasting habits through weekly practice.
• Track progress with a shared plan and check-ins.
• Leave the Client with a maintenance plan at term end.

Scope and Deliverables

The program runs across three phases over 12 weeks with weekly coaching sessions in the middle phase.

What is included

• 60-minute intake with written baseline plan
• Ten weekly one-hour coaching sessions
• Between-session check-ins on habits and progress
• Written maintenance plan at term end

Not included: medical diagnosis, treatment, or prescriptions; supplements or lab tests; and coverage between sessions. Missed sessions with less than 24 hours notice are charged in full.

Fees and Payment

The program is a fixed-fee engagement. Line items below map to the three phases in the scope of work.

Payment Schedule

Invoices are due within 15 days. Cancellations after week 4 receive a prorated refund of unused sessions minus a $250 administrative fee.

Acceptance

To proceed, complete the fields below and sign. This contract is valid for 30 days from the date on the cover.
